Home Care


We offer a variety of home care services to help with basic non-medical in home caregiving duties, such as light housekeeping, help with running errands and getting to appointments, and providing companionship.


  • This program exists so that members can obtain a consistent caregiver.

  • Our employees strive to work as a team and have the option to work independently to complete tasks, dependent on their position.

  • We work with our members’ caseworkers to create a unique care plan to fulfill the needs we can that are under our Scope of Service. We then follow the care plan and only complete tasks on the listed care plan.

  • We work hands-on with members and complete administrative tasks such as writing case notes, completing billing, creating schedules, and communicating with members’ caseworkers.

  • Supervisors and the Community Services Director meet once per week and the whole team meets multiple times per month to ensure our program is running smoothly and our staff and member needs are being met.

 

Location and Team Breakdown:

  • We currently serve Eau Claire, Chippewa, Dunn, and Winnebago County.

  • We have a total of nine staff members and are always looking to grow our team.

  • Out of the nine staff members, we have one Community Services Director, two Supervisors, five Specialists, and one Direct Support Professional. We are currently focused on hiring more Direct Support Professionals.

  • We currently have 20 active members who are receiving our services.
