This position is responsible for ensuring Housing Specialist are supported with their cases, monitoring for quality service, have positive working relationships with staff assigned. The Housing Supervisor is responsible for coaching, training and mentoring of staff. The Housing Supervisor is responsible for key metrics and following department processes. The Housing Intake Specialist is a remote position that will help support our Housing Counseling program by processing intake protocols and new referrals. This position does not specialize in housing individuals but helps new members to join our services.

Job description:

This position is remote and the candidate must reside in Wisconsin. Candidate must meet minimum requirements for proper work confidentiality and internet capabilities. There will be some travel requirements within the State of Wisconsin and this travel will include face-to-face contact with other agency staff and/or client-related matters. This position will be focusing on the Appleton, Neenah, or Menasha area.

Our Housing Counseling program assists clients with securing affordable, permanent housing. Clients may include low- to moderate-income families, veterans, a person experiencing homelessness, a person with a disability, or senior citizens. Many of these clients may be on government-assistance programs or are transitioning from temporary to permanent housing. Our Housing Counseling program performs housing searches, assists with applications and appropriate processes, contacts housing agents, and monitors a client’s progress.

Primary Duties Include:

• Professionally process all intake protocols for Housing Counseling and Housing Support Services with our partner agencies: MCO’s and County contracted agencies such as Inclusa, Lakeland Cares, IRIS, CLTS, CSP, 1915i

• Working directly with referrals in alignment with the Program Director or Program Manager

• Enter authorizations and manage utilization in the software management system with accuracy and timeliness

• Initiate setting up the client file in Share Point

• Run/Complete background check through appropriate programs and the State and National registries

• Work with member/guardian to complete an in-depth intake and assessment using the templates in Share point.

• Gather and confirm the birthdate, income information, household members, criminal history, rental history information, housing barriers, type of housing they qualify for

• Call current and past landlords for references using internal questionnaire script/document

• Collaborate with representative payees to determine the housing budget

• Case note all interactions and upload any documents in a timely manner

• Follow up phone call/meeting with referring care team to gain any additional information and address any barriers or concerns. Talk through the potential housing plan

• Create a housing plan/Person Centered Plan/Supportive Housing Plan/Goals depending on the barriers and referring agency

• Properly communicate goals and follow up email to request new authorization(s)

• Schedule a transition/’hand-off’ meeting with the Housing Specialist and their Supervisor

• Send a transition/’hand-off’ email to the care team/guardian/payee with the Housing Specialist and Supervisor’s contact information
