Job Overview
This position is based out of the Oshkosh, Neenah/Menasha community. Training will be required to have daily contact in-person, virtual and through phone support for up to the first 90 days. Training sessions may be hosted in Eau Claire, virtual, or in-person in Oshkosh.

Job Description
United Cerebral Palsy of West Central WI is looking for an Independent Living Program Specialist to provide quality services to clients by actively coordinating their care plan, completing home visits, and maintaining clear and steady communication with them and their care team to ensure needs are met and concerns are addressed. This position will require both hands-on and clerical tasks to be completed in support of client and program needs. The goal of this program is to assist individuals with disabilities to live independently in their community.
Primary Duties
Respond to referrals in an appropriate and timely manner
Communicate with case managers regarding client needs
Complete client intake and assessments according to the established processes
Develop goals and create a care plan for clients
Schedule and perform home visits, completing tasks according to the care plan, which may include accessing community resources and skill building
Maintain weekly or biweekly communication with the care team
Complete detailed case notes
Monitor and manage authorizations
Follow and maintain knowledge of established policies and procedures
Review assessments every 3 months, ensuring an informed and successful care plan
Maintain efficient work methods and production goals
Expectations:
Participate in staff meetings
Maintain an updated calendar including work schedule, meetings, deadlines, and events
Effectively communicate ideas and department needs using clear verbal and written communication
Ensure safe work practices

High school diploma or equivalent

Ideal Qualifications:
Associate’s or Bachelor’s degree
Work experience in human services field
Work experience providing direct cares

This position is responsible for ensuring Housing Specialist are supported with their cases, monitoring for quality service, have positive working relationships with staff assigned. The Housing Supervisor is responsible for coaching, training and mentoring of staff. The Housing Supervisor is responsible for key metrics and following department processes. This position will be working with other Housing Supervisors to be aligned with best practices. The Housing Supervisor must develop strong internal and external relationships. The Housing Supervisor supports the Housing Director and has routine and regular communications to support the growth of this service.
Responsibilities and Duties:
I. Process of Housing Services
a. Process authorizations for assigned staff
b. Process and ensure all data is accurate and complete
c. Review staff training and support needs
II. Complete with Accuracy & Attention to Detail
a. Client care plans and referring agencies have communication on progress
b. Maintain timely and accurate records and reports
c. Prioritize and efficiently handle tasks with deadlines
d. Attend and actively participate in weekly supervisory meetings
III. Maintain Confidentiality & Customer Service
a. Control sensitive and protected client and organization information in a highly confidential manner
b. Communication with clients is affective and respectful with all Staff Members, clients, vendors, and customers
c. Attend organization meetings held virtually and travel to meet customer needs
d. Provide staff feedback and evaluations
IV. Other duties as assigned by Supervisor
Qualifications and Experience:
Required
· High School Diploma or equivalent
· Knowledge and proficient use of computer software; third party software; Excel, and/or Windows Office/365
· Minimum of 1 year Case Management experience including assessment, care planning and follow-up
· Trustworthy and reliable
· Desire to be a supervisor

Preferred
· Minium 1 year of Case Coordination/Service Coordination in Family Carem, IRIS long term care or similar service models
· Knowledge of housing resources or work with marginalized individuals with focus on housing stability, lease obtainment and tenancy skills
· Knowledge of state, federal, local resources and public benefit programs that support individuals
· Obtained degree or currently enrolled at a University or Technical College pursuing a degree in Human Serivices or similar academic degree
